  1. Formative In-Depth Interview Research Guide (docx) A set of questions to help collect information from different people in the community about how pregnancies, births, and deaths are tracked.
  2. Screening Questionnaire (docx) A screening questionnaire for use in communities to learn about best practices for identifying pregnancies, births, and deaths.
  3. Telephone or Email Script (docx) A short script to use to request a brief interview with government health officials.

Baseline Assessment

  1. Inventory of Mortality Data Systems or Initiatives (docx) A table to help collect information about different systems or programs that record deaths and causes of death.
  2. Tool for Stakeholders (docx) A questionnaire for interviewing government and organization leaders who work with mortality data.
  3. Tool for Managers (docx) A questionnaire for interviewing managers of existing mortality data systems to learn how their platforms work.
